Mokins helps individuals and families explore in-home support from Registered Nurses, Licensed Practical Nurses, Home Health Aides and caregivers. We currently serve cities and communities throughout Will, DuPage, Cook and Lake counties in Illinois. Our team begins by understanding the person’s needs, routine, location and available coverage.
Mokins offers home-based support through Registered Nurses, Licensed Practical Nurses, Home Health Aides and caregivers. Depending on assessed needs, eligibility and the approved care plan, services may include nursing support, personal care, meal assistance, mobility support, companionship and help with daily activities.
RNs and LPNs provide skilled support in the home according to the individual’s needs, provider directions, applicable scope of practice and plan of care. The exact nursing services available are confirmed during intake and clinical review.
Home Health Aides support comfort, safety and independence by helping with personal care and everyday activities included in the authorized care plan. Support may include bathing, dressing, meals, eating assistance, toileting, transfers, walking, light housekeeping, laundry, shopping, transportation support, companionship and calling for help.
Caregivers may provide non-clinical personal assistance, companionship, supervision and support with daily routines based on the person’s assessed needs and approved service plan. A caregiver does not replace skilled nursing when clinical care is required.
